Transferring from one healthcare team or facility to another should be seamless. But sometimes in the hand off, gaps in the transfer can lead to gaps in care. Cheo Rose-Washington experienced this firsthand when a miscommunication between two hospitals left him with one of the rarest injuries in medical literature and nearly cost him his ability to walk. The reason so many handoffs between facilities fail? There is no standard framework for how they should work. Hand-Off exists to change that.
Hand-Off is on a mission to transform patient care by developing a solid framework for clean transitions between care teams and facilities. After partnering with Cheo Rose-Washington on his personal brand, we worked with him and the Hand-Off team to develop a brand and website that highlights their work and helps them connect with future partners.
A big part of our work was translating the complex concepts behind Hand-Off into a clear brand and a website that invites potential partners to learn more. Jessica Snitko, Ph.D, a research leader at Hand-Off, had this to say about the process: “The folks at Giant Shoulders are mind readers and magicians. Cheo and I would show up to a meeting and describe the vibe we wanted the site to have… We gave them half-baked ideas and, a week later, they would come back with a design that matched our vision perfectly.”
Congratulations to the Hand-Off team as they launch their vision for better patient care! You can learn more and check out their website at hand-off.ai.
